Documentation of any benefits received (SNAP, housing assistance, Social Security, disability, etc.)

Cornerstone Health Services
Cornerstone's charitable clinics provide completely free medical and dental care to uninsured adults in Tarrant County who have nowhere else to turn. All services are delivered by licensed physicians and dental professionals who volunteer their time, covered by liability insurance at no cost to them.
Charitable Medical Clinic
The clinic offers primary care and specialty services including dermatology, internal medicine, gynecology, orthopedics, and ophthalmology, as well as ongoing care for chronic conditions such as diabetes, hypertension, heart disease, and respiratory illness. Community partnerships extend access to lab work, imaging, and well-woman screenings. Patients may also be connected to health education, nutrition, and fitness resources.
Services are available by appointment in English and Spanish. Patients must meet eligibility requirements — read about patient requirements and submit an application (en Español) along with supporting documents.
Cornerstone also partners with UT Arlington, TCU, UNT Health Science Center, and Tarrant County College to provide clinical training and volunteer opportunities for healthcare students.
Cornerstone Charitable Clinic believes in not only meeting immediate health needs, but also in educating patients on how to take personal control of their health. Ongoing care and consultation is available for patients learning to live with high blood pressure, respiratory ailments, heart disease, and diabetes.

Charitable Cataract Clinic
The cataract clinic is the nation's first free cataract facility for the uninsured. Volunteer physicians and ophthalmic staff have the ability to treat up to 10 patients per clinic day with the full support of Cornerstone's trained medical staff. The Cataract Clinic gives hope and provides solutions for patients with loss of vision and no ability to pay for sight-restoring procedures. Thanks to the generosity of Alcon Pharmaceuticals, the Clinic provides all necessary equipment and medications needed by physicians and patients. For more information, call 817-632-6035.
Dental Clinic
Cornerstone Dental Clinic provides comprehensive oral exams, radiographs, fillings, cleanings, extractions, partials, crown/bridge, and root canals. We do not provide orthodontic or oral surgery services. Patients are seen by appointment only; no urgent/emergency services are available.
You must submit an application , Formularios de solicitud along with supporting documents. To qualify, you must be: Tarrant County Resident, have no insurance, and fall below 200% of the Federal Poverty Level.
Applications can be dropped off at our office (3500 Noble), faxed to 817-632-6026, or emailed to dentalclinic@canetwork.org. Applications are processed in the order they are received and may take up to 6 weeks to review for eligibility. Once approved, you will be scheduled for the next available comprehensive exam appointment, typically 4 weeks out.
